Parents,
As the new school year begins, I would like to clarify and explain our school's medication administration practices. These are based on guidelines from the Kansas School Health Manual prepared by the Catholic School Nurses Association.
1. All medication to be administered will be in an official prescription container and be accompanied by a written request from a physician.
2. The physician's request form should identify the medication, dosage, reason for the medication, and time it should be given.
3. A written request form the parent/guardian should accompany all medication. There is a section on the medication request form for the parent to sign.
4. Any changes in the type of drugs, dosage, and/or time of administration should be accompanied by a new physician and parent permission signature and a newly labeled pharmacy container.
With the above guidelines in mind, we can not accept ANY medication unless it is in the official prescription container and properly labeled with instructions on how it is to be given. Additional bottles with prescription labels can be obtained at your pharmacy. A parent should bring the medication to the office and notify the nurse or office manager that the medication is available and how and when it should be given. Medications should not be sent with the student.
In the past, we have been asked to keep "extras" on hand in case a child has forgotten to take their medication. We will no longer be able to do this. If the parent or child believes a dose was missed, the parent/guardian will be called and asked to come to school to give the missing dose. This will hopefully eliminate the inadvertent overdosage of a medication.
